Eligibility Questionnaire
Use this questionnaire if:
- Your client is under 70 years old and wants to insure a trip cost of more than $25,000; or
- Your client is over 69 years old and wants to insure a trip cost of more than $15,000; or
- Your client is over 69 and under 90 years old and wants to buy the All Inclusive Worldwide Plan for more than 16 travel days.
